OFFICE VISITSOffice hours are by appointment only. As a referral center,
priority is given to patients who are referred for consultation by other
physicians. We make every effort to keep to our appointment schedule. Unfortunately due to the nature of our surgical specialty, life-threatening emergencies can arise that may cause major and minor delays within the schedule. We apologize for any inconvenience that may cause. Please extend us the courtesy of notifying us 24 hours in advance if you need to cancel or reschedule your appointment. What to bring Please bring a complete written list of your medications to each office visit. This is especially important if you are taking several different medications prescribed by other doctors. If there is an ear problem, an audiogram may need to be done unless you bring the results of a recent (within 6 months) test. Relevant past medical records, specifically summary notes from other ENT doctors, ENG reports, ABR and ECoG reports, past CT scans and MRI scans (Please bring the actual films as well as the written report) are usually helpful. Please try not to bring large stacks of irrelevant paperwork. Please present your Driver’s license and insurance card at the time of your visit. Not only is the driver’s license a form of identification, the picture helps identify the medical record. Intake Patient Information Forms, and Consent Forms: You may download and print the intake forms from the website if you like. This gives you time to go over the forms in the comfort of your home, instead of feeling overwhelmed by all the intake paperwork. You may fill out and sign the paperwork ahead of time. If you are uncomfortable signing certain items, please feel free to ask questions when you arrive. Don't forget to bring the completed or near completed forms to your appointment! Comprehensive Authorization Sheet Health Care Arbitration Agreement IMPORTANT INFORMATION What happens if Dr. Li is not on my insurance? You may still see Dr. Li. If you have out of network benefits: You will be asked to pay for the visit at the time of service and then, your insurance company will reimburse you later. As a courtesy, we will file your insurance for you. If you are on an HMO plan, you have the following options: You may cover the cost of the visit yourself…. or you may obtain a referral from your primary physician based on a special need in a underserved specialty. (There are very few otologists and neurotologists in Florida). We appreciate and encourage payment for services at the time of your visit. Patients are responsible for deductibles and co-pays at the time of service. If you have questions regarding our fees, please feel free to speak with our Office Manager. * Please note: We accept fee assignment of Medicare claims. These claims will be submitted directly to Medicare by our office.
